Well, I kind of imagined that when Elementals were inside their element and they wanted to look out, all you might see are a pair of eyes. In the case of fire elementals, they could be burning embers; Earth Elementals might possess black coals, deep pits or jewels; Water Elemental eyes might appear like spheres of foam etc. The example of their face breaking the surface was at the time the easiest way I could think of describing it while I was at work.
If they did manifest some part of themselves outside their element then I completely agree there should be some kind of sign. But it should be in direct proportion to how much they reveal. An elemental that pokes it's baby toe out should get a much larger bonus to its hide check then an elemental who sticks its head and torso out of the ceiling. In fact, improved cover gives a +10 bonus to hide and improved evasion for reflex saves!! Toot is almost always hiding so that's really good to know.
