Ursk readies an  attack and hit the skeleton: 
Attack: [1d20+6] = 16+6 = 22 damage: [1d10+3] = 3+3 = 6
"What is that?" Taijun says with surprise at seeing the four armed skeleton coming at them from the dark. He plants his feet reading to kick the shambling bones if it gets past Reawyn's bomb or Ursks axe.

It's a skeleton with four arms - Evandroid answers  Smile
The strange charges in faster than anyone can react. It goes straight for Ursk. The dwarf sidesteps and swings with his axe. The axe mostly passes through without doing much damage to the creature.
so now initiatives?
Initiative is already taken care of. Everyone but Ursk has actions.
